The accolades recognise the top performers in each position in the tournament.
Meanwhile, Europa League holders Manchester United provided the final three nominees for the second-tier continental competition's Player of the Season prize.
Swedish hitman Zlatan Ibrahimovic, who left United at the end of last season, will compete with Henrikh Mkhitaryan and Paul Pogba for the gong.
The winner will be announced at the UEFA Men's and Women's Player of the Year awards ceremony on August 15.
